The Bureau of Family Health invites you to help influence Pennsylvania’s Title V Maternal and Child Health Services Block Grant

The Bureau of Family Health requests your voice to help identify the seven to ten priorities that will drive Pennsylvania’s Title V programs.

This survey is an opportunity for parents and caregivers, families with special needs, birthing people and their partners, youth, maternal and child healthcare professionals, and organizations across Pennsylvania to share their thoughts and perspectives on the priorities that will drive Pennsylvania’s work through the Title V Maternal and Child Health Services Block Grant over the next five years. The results of this survey and input from a series of regional meetings and virtual events will inform the selection of the final seven to ten priorities.

Please note that this survey is for people who have not yet had an opportunity to rank the potential Title V priorities. If you have already attended one of our regional or virtual events, we ask that you do not complete the survey as we have already received your input. We appreciate you sharing your voice.
